1. Proper COVID-19 protocols including mask wearing and social distancing are not in place or enforced at the facility. 2. At least 10 COVID-19 positive cases have come from the facility. 3. No measures have been taken to limit employee exposure to the public. 4. Employees are required to report to the office even when work could be accomplished from home. 5.
