1. Not taking daily temperatures of all personnel as mandated by COVID requirements. 2. Masks are not being enforced to be worn by ALL personnel. Shared offices are not following protocol for wearing masks. Leadership is not showing commitment to ensure protocols are being followed throughout the day. Tours have taken place under the direction of the leadership team with protocols not being followed. Not wearing masks or social distancing. Masks are not being worn in restrooms or locker rooms. Workers who have had Covid and return to work are not wearing masks. 3. Routine sanitizing of surfaces does not take place. 4. Social distance in break room is not being followed and there is no monitoring of this area for compliance. Separation barriers in the break room are not present. Break room is NOT routinely sanitized. 5. Contractors entering facility daily from various points of entry not getting checked for temp and they do not wear masks while working or walking through common areas. Not maintaining 6 ft social distance. 6. There is no routine communication on the outbreaks that are occurring in the facility.
