Destiny Dental continues to be in violation of and non-compliant with the following Infection Control Standards: Blood Borne Pathogens Standards, 29 CFR 1910.1030, 1910.1030 (g)(2); PPE Standard, 29 CFR 1910.132; Respiratory Protection Standard, 29 CFR 1910.134 as follows: Dental Assistants & Dentists: 1. Assistants do not thoroughly disinfect the operatories between patients. 1910.1030 (d)(4)(ii); Blood spatter was left on the overhead light and other items from prior patient(s). 2. Assistants (sometimes the dentists) do not wear lab coats nor gowns. Not even during procedures! Often times, the assistants (and the dentists) do not wear face masks or safety glasses during procedures and exams. 3. Assistants and Dentists have been seen touching things around the office while wearing dirty gloves. 4. On a daily basis, the assistants will team up to do X-rays. One assistant (without a vest on) will be in the room with the patient and the other will be in the hall pushing the button to run the X-ray machine. Anywhere between a single image to an 18 image FMX. 5. Assistants and Dentists go into the reception and break areas with contaminated scrubs, the occasional lab coat, and sometimes even dirty gloves on. 6. Employees are never observed washing their hands or using hand sanitizer. Sterilization Lab: 1. The dirty side and the clean side of the lab are both dirty because everything is touched with bare unwashed hands, or dirty gloved hands by most employees. 2. Instruments are never put into pouches before being run through the autoclave (sterilized). They are run through the autoclave, then put into pouches AFTER they go through, and then put into drawers with non-gloved hands OR left on the counter top for everyone to touch when assistants are looking for the instruments they need for their procedure tray. 3. Many disposable items are being re-used (e.g., the tips on the etch and composite syringes, impression trays, etc.). 4. The only sharps container is kept on the countertop in the lab and is not secured to a wall. It is always full! Laundering PPE; 29 CFR 1910.1030 (d)(4)(iv): 1. There are no on-site laundry facilities. Training; 1910.1030(g)(2): 1. Orientation, initial or beginning employment training is/has not been performed. The assistants train each other. 2. Annual training is/has not been provided to employees on anything (e.g., hazards, exposures, fire safety, BBP, etc.). The only "training" that has been provided is a 10 question online quiz that employees are supposed to take monthly through eagleassociates.net on their own time. The office manager completes this for both of the dentists. General: 1. It is believed that the office manager doesn't have the required paperwork for the COVID-19 preparedness response plan, the exposure control plan, the engineering and work practice controls. 2. Hepatitis B vaccinations were not made available to employees, including the paperwork. 3. There are not enough personal protective equipment supplies, including face masks available for employees. Employees are expected to wear what they are provided for the entire day between patients. 4. There are no emergency lights, lit "Exit" signs, an escape route in case of fire, or accessible fire extinguishers. The employer may not have a fire alarm
