1. Facility allowed a new resident to move into the building during the state ordered "lockdown", the resident came from a nursing home where COVID-19 cases had been determined and she herself had COVID-19. They did not test this resident prior to moving her in. 2. Masks were not being distributed to workers and staff. The director of nursing wasn't allowing anyone to wear a mask during this time and the time of the resident moving in. 3. There was no reassurance during this time of moving the resident in when staff expressed concerns daily of this resident possibly having COVID-19.Source: Osha.gov | Receipt Date: 2020-04-15
