1. Water lines for machines for equipment are in need of repair and known to cause water to contact electrical panels. 2. Flexible electrical cords are used to supply power to equipment in lieu of permanent wiring. 3. Improperly grounded fans and equipment are located and used within the facility. 4. Exposed wires on fans and various equipment are located throughout the facility. 5. Overhead cranes used at the facility have not received required annual inspections. 6. Induction heat machines operate in automatic modes while not effectively guarded. 7. Building #2 contains an exhaust fan on equipment that is in need of repair and not performing its function of exhausting hazardous fumes. 8. The employer does not have a program for the control of hazardous energies (LO/TO). 9. The employer is not following State of Ohio mandates and CDC guidelines with regards to protecting employees from Covid-19.Alleged Hazards: 9, Employees Exposed: 50
