1. The employer has not effectively developed or implemented an Infectious Disease Preparedness and Response Plan for COVID-19. Masks or face covering are not properly used and the use of masks is not enforced. Contact tracing has not been conducted with employees that has been in close contact with positive COVID-19 cases so they either could quarantine or get tested. Four employees exhibiting symptoms are working. Social distancing is not maintained at the location. There have been six positive COVID-19 cases in the last week. 2. The employer has not provided adequate sanitization at the facility such as, common areas and work areas, after confirmed COVID-19 positive cases.
