1. There were 10 positive COVID-19 cases last week. Contact tracing is not done, and employees in close contact with employees who tested positive, are not quarantine or send to be tested exposing other employees to the virus. 2. Social distancing is not maintained when clocking in or clocking out. Employees stand next to each other and six feet distance is not maintained. 3. Employees are working next to each other in the "gift wrapping", "training" and "hospital" areas. There are no work stations and social distancing is not maintained. 4. The use of mask is not enforced. There has been no masks in the facility for the last week.Alleged Hazards: 4, Employees Exposed: 20
