1. Employees who clean the bathrooms are not provided personal protective equipment. 2. Bathrooms are not adequately cleaned. 3. Mud and water is frequently present on work room floors which creates slipping hazards. 4. Employees transport very heavy objects such as rolls and machine parts. The employer does not enforce the wearing of protective footwear. 5. The employer does not provide protective gloves for employees working on production lines where cut hazards are present. 6. The employer has not implemented a COVID-19 prevention program including good housekeeping practices of regularly cleaning and disinfecting surfaces, equipment, and other elements of the work environment. 7. The employer has not implemented a COVID-19 prevention program including wearing face coverings and or not wearing face coverings properly. 8. The employer's COVID-19 prevention program is not adequate in that it does not have adequate screening to ensure potentially exposed employees are separated and sent home. 9. The employer has not implemented social distancing as part of a COVID-19 prevention program.Alleged Hazards: 9, Employees Exposed: 250
