1. Employees are not wearing face coverings in accordance with Pennsylvania Department of Health and Centers for Disease Control and Prevention (CDC) guidelines for enforcing the use of face masks. 2. Employees are not social distancing in accordance with Pennsylvania Department of Health and Centers for Disease Control and Prevention (CDC) guidelines for controlling the spread of COVID 19. 3. Employees are not following the protocol when exposed to an employee that tested positive in accordance with Pennsylvania Department of Health and Centers for Disease Control and Prevention (CDC) guidelines for controlling the spread of COVID 19. Not everyone that was exposed to an employee that tested positive got tested or quarantined and some people came back early from being quarantined. 4. Employees are exposed to trip hazards due to poor housekeeping issues with tools and debris collecting in the walkways. 5. Employees are exposed to hazards due to poor housekeeping issues with dust and debris collecting under machines and throughout the facility.Alleged Hazards: 5, Employees Exposed: 1000
