There is no physical distancing of chairs in waiting room in accordance with Pennsylvania Dept. of Health and CDC recommendations and guidelines. 3. The physician(s) refuse to wear masks and made masks optional for staff which is not in accordance with Pennsylvania Dept. of Health and CDC recommendations and guidelines. 4.
