Evergreen and dementia units: 1. The employer did not follow Pennsylvania Department of Health and Centers for Disease Control and Prevention (CDC) guidelines for informing employees of their possible exposure to COVID-19 in the workplace. This facility did not isolate patients but kept them in the same double rooms as residents who did not have COVID and later those other roommates acquired COVID. This occurred on their dementia unit as well. Staff were on the Red Zone not wearing masks or proper PPE. Staff take care of both COVID patients and non-COVID patients.Alleged Hazards: 1, Employees Exposed: 20
