1. Within the facility, at least three (3) employee has tested positive for coronavirus disease 2019 (COVID-19) and the employer is not following the current CDC Guidelines, such as but not limited to, the Interim Guidance for Businesses and Employers to Plan and Respond to Coronavirus Disease 2019 (COVID-19). 2. It is alleged the employee who tested positive and/or are exhibiting signs and symptoms of COVID-19 did not follow the protocols for quarantine, employees who have come in close contact with COVID-19 positive staff are not notified, are not directed to quarantine for the recommended amount of time and/or follow other available guidance for exposure. 3. Proper disinfecting, sanitizing, and cleaning is not being performed in accordance with CDC Guidelines, especially after positive COVID-19 employees and/or patients have been in the area. These areas include, but are limited to, high touch areas, offices, restrooms, common areas, and workstations.Alleged Hazards: 5, Employees Exposed: 15
