1. The employer failed to ensure that high-hazard procedures were conducted in Airborne Infection Isolation rooms. At approximately 10:15PM on January 1, 2021, a COVID-19 case was transferred to the Intensive Care Unit, Room 2210, where they received high flow nasal cannula oxygenation. This room was not an airborne infection isolation room or a room converted to operate under additional filtration and negative pressure. Employees in the nursing station approximately 8 feet from the room door were not provided respiratory protection to protect them against any infections aerosol ejected when the door was opened and closed. T8 CCR 5199(e)(5)(C) 2. The employer failed to ensure that respirators are provided and used in accordance with NIOSH approval. Employees are only provided one N95 respirator each day, are required to wear it for up to 16 hours, and don and doff the respirator more than 20 times.
