1. Employees are utilizing the wrong types of ladder for electrical work, i.e. aluminum ladders are being used. 2. Employees on the job site(s) are not properly trained to perform live electrical work. 3. The employer does not provide employees with any personal protective equipment for live electrical work. 4. The employer does not provide employees with any personal protective equipment for job tasks, i.e. safety glasses or hearing protection. 5.
