1. Employees are not following the safety rules. 2. Malfunctioning equipment (heat sealers) are catching fire and shocking the staff. 3. Employees are constantly running out of biohazard bins. 4. Mask are not strictly enforced by management for the donors. The agency does not have jurisdiction or standards to manage the following complaint items: 5. Donors out number the phlebotomists.
