1. Forklift drivers are not driving in a safe manner. Employees are not trained to use forklifts. 2. The pallets are of an incorrect size such that they do not stack onto the racks neatly and the product protrudes. Employees do not receive training on the proper way to stack pallets with product. 3. Employees do not receive training on the safe use of box cutters. 4. Steel toe shoes and safety glasses are not made available to employees. 5. The warehouse is too hot. The swamp coolers are broken, and no air conditioning is supplied. 6. Work-related injuries are not reported to OSHA.
