1. The spray bottles used to clean rooms are not labeled. Cleaning is not done on a regular basis between patients. 2. No social distancing is being practiced. Employees share the breakroom and do not maintain 6-feet of distance. 3. The masks are not readily available to employees and are locked up. Staff is not wearing masks or gloves within the clinic. 4. Thermometers are oral and not forehead type. 5. An employee from Physical Therapy allegedly tested positive for covid-19, but no other information has been provided to employees.
