1. No Employee safety training is conducted which has increased the rate of injuries on the production floor. 2. Materials such as lumber and wire has incorrectly been disposed of by either human or machinery throwing it off one production line to fall into another line . This has allegedly happened on several occasions which has caused injury to employees when debris from wooden wirebound crates falls on employee working on other lines on the production floor. 3. Employees are allowed to wear earphones, ear buds/pods underneath regular earmuffs designed for safety, This creates a safety risk due to employees due to unable to hear for emergencies slows the reaction to events such as fires and emergency shutdowns. 4. Covid Related Complaint - Mask Wearing is not being enforced in both the administrative offices and production floor respectively. no employee safety policy has been established to instruct workers on path of travel in facility and no social distancing protocols have been implemented in facility. 5. Roof on top of production floors is in a state of disrepair causing roof leaks which can create slip, trip, and fall hazards for employees during shifts .
