1. There have been several cases of COVID-19 in the facility and the employer did not inform exposed employees. 2. Approximately one month ago, the employer failed to stop machine operations due to lack of qualified personnel. Line 4 was operated with only two qualified personnel and two other employees from a different line. Neither of the other two employees were qualified to operate the machines.
