1. The employer did not develop and implement an Infectious Disease Prevention and Preparedness Program for COVID-19 including but not limited to social distancing and the use of face covering. Upon knowledge of a positive test at the workplace, the employer did not identify and quarantine workers that might have been exposed. 2. The employer has not provided adequate sanitization in the facility, common areas and work areas after confirmed COVID-19 positive cases.
