1. One month ago in the Conference Room, 20 employees were given training over two days for four to six hours per day. The two trainers and the employer's COO did not wear face coverings in accordance with Pennsylvania Department of Health and Centers for Disease Control and Prevention (CDC) guidelines. The 20 employees were unable to practice social distancing in the Conference Room in accordance with Pennsylvania Department of Health and Centers for Disease Control and Prevention (CDC) guidelines. The Conference Room was not ventilated in accordance with Pennsylvania Department of Health and Centers for Disease Control and Prevention (CDC) guidelines. One of the trainers tested positive for COVID-19 and showed symptoms during the training potentially exposing the 20 employees to SARS-CoV-2. Subsequently, 13 employees have tested positive for COVID-19. 2. Following the close contact with the COVID-19 positive trainer, employees returned to work before they met the criteria to discontinue isolation in accordance with Pennsylvania Department of Health and Centers for Disease Control and Prevention (CDC) guidelines potentially exposing workers to SARS-CoV-2. 3. The employer does not enforce the use of face coverings by all employees, does not disinfect frequently touched surfaces, does not screen employees for symptoms of coronavirus prior to the beginning of their shifts, and does not enforce social distancing in the lunch room in accordance with Pennsylvania Department of Health and Centers for Disease Control and Prevention (CDC) guidelines potentially exposing workers to SARS-CoV-2.
