There is no policy in place to provide employees adequate space to ensure 6 foot distance is possible. There is no health screening done on employees or patients that come in the facility on a daily basis. No deep cleaning was done in July when this facility had 2 employees test positive for COVID-19. No written guidelines in place to handle COVID-19, including cleaning, etc. Employees are not being trained properly on how to deal with COVID-19.Source: Osha.gov | Receipt Date: 2020-11-18
