1. Mold is on the ceiling and pipes in several rooms on the first floor. 2. Sheets with feces are in the first floor utility room sink. The sheets has been in the sink for months. 3. Sharp containers on the first floor are over filled. 4. It has been months since the last time the building was sanitized for COVID-19. 5. Dead bugs are in the corners next to the beds; and feces is on the lower walls next to the beds in rooms on the first floor.Alleged Hazards: 5, Employees Exposed: 20
