1. COVID-19 positive patients are admitted to the facility. 2. The employer did not implement and develop an OSHA guidance based "effective" workplace COVID-19 prevention programs to include conducting a hazard assessment, addressing safety protocols such as isolating employees with symptoms at work, cleaning and disinfection practices, training, screening and testing, and recording and reporting cases. 3. The employer did not provide adequate isolation personal protective equipment such as adequate respiratory protection-N95 or surgical masks and eye protection to employees in contact with COVID-19 residents. 4. Isolation equipment was placed inside the room of positive COVID-19 resident, so employees had to come within 6 feet to dawn protective clothing. 5. Social distancing guidelines were not followed or encouraged in the facility. 6. Isolation and quarantine practices were not implemented or followed. 7. The on site hand sanitizer was not alcohol based. It was not an EPA registered disinfectant product qualified for use against SARS-CoV-2.
