1. PPE, such as gloves and facemasks, are not supplied to workers who are caring for people in the day care and may be exposed to COVID-19. 2. Rules regarding use of face coverings for workers and others who enter the facility are not enforced, exposing workers to COVID-19. 3. Employees who have symptoms of COVID-19 continue to work in the facility, exposing their coworkers. 4. Employees who have travelled recently are not required to follow guidance of the state and CDC to quarantine. 5. The establishment is not maintained in a sanitary condition and is not cleaned according to CDC guidance despite having had staff and clients having tested positive for COVID-19 or exhibiting symptoms of COVID-19.Alleged Hazards: 5, Employees Exposed: 20
