1. Some machines including shearing, bending, stamping, cutting, and others, do not have proper operator safety guarding. Three to four months ago, a worker had part of his finger cut off while operating a machine and was taken to the hospital, this was not reported to OSHA. 2. The director of engineering placed a 3D printer in the engineer area without any ventilation at the inspiration lane location. 3. Several managers refuse to wear face coverings/masks when walking in hallways, when in meetings, and in the cafeteria. 4. Ten to twelve employees work in a small room (~500 sq ft) and are not required to wear face coverings/masks while sitting.
