The employer did not report a serious illness of an employee to the nearest District Office. T8 CCR 342(a). The employer did not establish, implement and maintain an effective written ATD Exposure Control Plan in the following instances: 1. The employer did not ensure that employees maintain spacing of at least 6 feet while working at computer terminals or install cleanable solid partitions that effectively reduce the aerosol transmission between the employees. T8 CCR 5199(e)(1). 2. The employer did not report an outbreak of at least three or more COVID-19 cases in a week immediately to the local public health officer. T8 CCR 5199(h)(6)(A). 3. The employer did not notify employees who had significant exposures of the date, time and nature of the exposure. T8 CCR 5199(h)(6)(C)(2). 4. The employer did not ensure to implement and maintain an effective procedure for reducing the risk of transmitting aerosol transmissible diseases by not allowing enough space to employees to maintain a distance of 6 feet from each other at all times.
