1. The employer failed to clean and disinfect areas, material, and equipment used by a COVID-19 case during the high-risk exposure period. There has recently been a COVID-19 outbreak within the facility, and employees are not made aware that cleaning and disinfecting has taken place. T8 CCR 3205(c)(8)(C)3. 2. The employer failed to exclude employees, for a minimum of 10 days from the workplace, who were exposed to a COVID-19 case while working on the 1st, 2nd, and 3rd floor. T8 CCR 3205(c)(10)(B) 3. The employer failed to implement cleaning and disinfecting procedures. The employer has never cleaned and disinfected the waiting room. T8 CCR 3205(c)(8)(C)1. 4. The employer failed to ensure that all employees are separated from other persons by at least six feet, except where an employer can demonstrate that six feet of separation is not possible, and except for momentary exposure while persons are in movement.
