1. Outside workers with gloves on are putting mask on j hooks. 2. A napkin dispenser is being used to dispense mask. 3. Mask are stored in the dispenser and employees are expected to grab one when needed. 4. Not all employees are washing their hands prior to touching the mask. 5. If employees ask that a mask be hung on a hook away from employees wearing gloves, they are refused a mask.Alleged Hazards: 5, Employees Exposed: 25
