1. COVID-19 protocol not being enforced by management. 2. Staff not being made to wear masks despite multiple complaints. 3. Sick crew allowed to remain at work infecting others. 4. Not alerting staff to COVID-19 exposure. Employee caught COVID-19 from a coworker who was obviously sick and took off their mask 2 days in a row and coughed by other employees. They allowed the employee to remain at work despite being visibly ill. 5. Employees not being truthful on their daily COVID-19 screening. 6. After being informed of close contacts by COVID-19 positive employee, management did not alert or quarantine any of the employees, they did not require the sick employee to quarantine or request a COVID-19 test.Source: Osha.gov | Receipt Date: 2021-01-08
