1. Employee tested positive for COVID-19, the facility was never professionally cleaned. 2. High touch areas have not been wiped down since June 30. Employees tested positive around the second week of July and no precautions were taken. Employees were told to report to work with masks. 3. Employee that tested positive was around high touch areas. Employees that were in direct contact with the person that tested positive were not notified even though everyone worked together in an office setting, where masks were not made mandatory to wear.Source: Osha.gov | Receipt Date: 2020-07-27
