1. Employees are not properly being screened to enter the facility. 2. Employees screen themselves by filling out a form. 3. No screening of employees upon re-entry into the facility. 4. Staff told "carefriends" that an employee had covid-19; one positive test and one negative test. 5. The facility refused to test everyone in contact with the positive employee. 6.
