Hazard Location: Manufacturing Floor 1. Slippery warehouse floors- Oil from machine press spread throughout work area floors. 2. COVID-19- Employees on 2nd shift do not wear mask, exposing each other to possible COVID-19 virus. 3. Forklift equipment driver- does not adhere to safety precautions within the work area; resulting in 2 accidents. 4. Machine Press- exposed bare wires stick out which could possibly lead to electrical shock or press jamming up on employees.Alleged Hazards: 4, Employees Exposed: 10
