Item 1: The employer is not implementing adequate safety practices to protect its employees from the spread of SARS-CoV-2 (the virus that causes the disease COVID-19) such as, but not limited to: a) Providing or requiring employees to wear respirators or face covering when working around residents or other employees that have tested positive for COVID-19 b) Screening employees for symptoms of COVID-19 or taking temperatures prior to entering the workplace c) Ensuring cleaning or sanitizing procedures are followed for work areas and high contact surfaces (such as hand rails, light switches, etc.) Item 2: A former-tenant's room (House 3) was soiled with urine, feces and other bodily fluids. The carpets and floorboards in the room are still soiled. The carpet was never professionally cleaned and the room still smells. Employees kneel and work on the floor to assist the current tenant in that room. Item 3: The floor in the dining room of House 1 unsafe and squeaks and flexes when walked on. Item 4: The kitchen sink drain in House 1 leaks and a bucket is used to catch the leak. The bucket and space under the sink is rancid because of the collecting water.
