1. The employer has not effectively developed or implemented an Infectious Disease Preparedness and Response Plan. Employees and management are not wearing masks consistently. No social distancing is maintained in the break room with 15-20 employees sitting next to each other. 2. The chain in the conveyer belt is constantly malfunctioning (breaking up) and not fixed.
