1) The trap under the sink in the lab is continually clogging; requiring frequent manual clean-out of plaster model trimmings by employee(s), which poses unnecessary risk of COVID infection. 2) Engineering & work practice controls for preventing environmental surface contamination are getting lax such as keyboard cover being removed during use and cross-contamination of drawers / cabinets by handling with contaminated gloves. 3) There is a leak under the tile floor, causing slipping hazard of the water pooling on the floor. 4) Bloodborne Pathogens infection control training of staff is not conducted annually.Alleged Hazards: 2, Employees Exposed: 4
