As it pertains to both the Shelton and Fairfield Locations: 1. It is alleged that at least seven (7) employees have test positive and all employees were not notified at all and/or within a timely manner of a potential indirect and/or direct coronavirus (COVID-19) exposure or encounter. 2. Employees working in close proximity to each other and patients are not practicing social distancing in accordance with CDC Guidelines when working and moving throughout the facility. 3. Proper disinfecting and cleaning is not being performed in accordance with CDC Guideline, including high touch areas, and after having several positive COVID cases with the facilities. 4. Owners and employees are not are not provided with personal protective equipment (PPE) such as, but not limited to, masks; and/or they are not directed to wear PPE within the facility. It is alleged that staff, including owners, do not wear PPE such as, but not limited to masks. 5. CDC Guidelines, such as but not limited to, the Interim Guidance for Businesses and Employers to Plan and Respond to Coronavirus Disease 2019 (COVID-19) are not being followed. It is alleged that the devices used for taking the temperatures of staff and patients are not properly calibrated. note there have been three (3) complaints filed against the Fairfield location since September 21, 2020.
