1) Employer is not following the Center for Disease Control protocols for COVID-19 prevention including mask wear, social distancing, cleanings, and quarantine procedures. 2) Employer forces employees to sit near shoulder to shoulder in meetings and many people do not wear masks. 3) Management has routinely joked about COVID-19 and does not take seriously and ridicules employees who wear masks. 4) Ten employees at the worksite have contracted COVID-19 and one employee has died.Alleged Hazards: 4, Employees Exposed: 50
