1. Staff are being required to work without having test results for Covid-19. 2. Employees are forced to wear contaminated masks. On one occasion PII a trainer did not have access to clean masks and employees had to provide their own. 3. Employees are frequently lowering their masks to their chins throughout the building. 4. Empty hand sanitizing stations throughout building. 5. Residents medications are being dropped on the floor and still being given to the residents. 6.
