1. On July 29, 2020, an employee working at the facility developed symptoms of Covid-19. On July 31, 2020, the employee notified their supervisor that they had tested positive for Covid-19. The employer choose to remain open to provide services to clients. 2. There was no mention of a deep cleaning process taking place due to the covid-19 positive staff. 3. Remaining staff were required to be tested for Covid-19 on July 31, 2020. Medical professionals clearly warned Lighthouse Autism Center administration that the staff were being tested too early and therefore, the test results could be inaccurate. 4. Lighthouse Autism Center administration failed to notify a staff member, who no longer works for the facility, that they had been exposed to a Covid-19 positive staff person the previous week. 5. The facility has 20 adult staff providing services to ten children. These children are not required to wear masks.Source: Osha.gov | Receipt Date: 2020-08-18
