1211 Stateline Road, Niles, 49120 Michigan, United States

On May 2nd, before employees began their shift they were informed that 22 out of the 87 residents present at the site were COVID-19 positive. Upon beginning their shift, an employee was not provided either a N-95 mask or face shield even though they were working directly with positive residents. The employee was only provided a cloth mask, and they were informed that they did not have enough face shields for everyone. However, management was provided N-95 masks. An employee subsequently tested positive from working that day because they were not provided the appropriate personal protective equipment.

I don't know what it is or why I received it,Shipper re#000236****. It's first class mail so I'm trying to figure how important it is also I've had some problems with identity theft which is the main reason for my concern.
