COMPLAINT ITEM # 1 - Employees have been exposed to an employee who tested positive for COVID-19, but not all employees have quarantined as some still work in the store. COMPLAINT ITEM # 2 - The facility has not been deep cleaned to sanitize high touch areas such as restrooms, without documents confirming deep cleaning to employees upon request. COMPLAINT ITEM # 3- The air vents are still covered in dust and dirt resulting in an unsafe work area where dust falls onto employees and into wax pots while attending to clients. COMPLAINT ITEM # 4 - Employees are exposed to a manager who will not fully quarantine after being exposed to a COVID positive employee. COMPLAINT ITEM # 5 - Employees are not informed of COVID positive exposures within the facility initially and if they are, their identity is divulged.Alleged Hazards: 5, Employees Exposed: 10
