1. Failure to comply with social distancing measures by maintaining a minimum six-foot distance between employees in public spaces. Employees are not social distancing when clocking in and out at the time clock. 2. The employer is not providing personnel to disinfect the kitchen and cafeteria areas. Employees and clients dining maybe exposed to Covid-19. 3. Possibly contaminated left-over trays and utensils from Covid-19 positive patient rooms are being brought to be cleaned when the patients leave. The items are mixed with other patient trays and utensils and workers may exposed. 4. Utility workers are not provided adequate masks/face coverings. The masks provided are too small, possibly made for children and do not completely enclose the nose and mouth. Hazard Location: 1. Time clock is in hallway in front of Chef's office by kitchen area. 2-4.
