4649(a): Air gauges in front tire bay not working properly. 4649(a): Air Bazooka- has no air gauge, employees are modifying own plug & valve.(mounted on pillar on wall in tire bay). 3325: No safety cages in bays for tires to be aired up or checked(Front tire bay). 3203: Chains across alignment pit are broken(Alignment pit & upstairs tire warehouse). 3668(a): Forklift operators not certified. 3663(f): Ignition does not work on the forklifts (back of shop). 4649(a): Air tanks have not been serviced in years (back of shop by tire storage). 3203(a): Training of any kind is not provided. 3273: Housekeeping issues (meeting room, locker/lunch room upstairs). 3203: Torque wrench not inspected/documented. 3542(d): Car lifts not serviced by a certified technician (all lifts in shop and tire bays). 3205(c)(1) / 3205(c)(2): 4 employees in November tested positive for COVID-19, shop was never cleaned or quarantined. 3328: Employees working on equipment and large truck tires that should be T.I.A certified, (truck shop bay 6). 3203: No tire dollies to help lift heavy tire & wheel assemblies (front tire bays). 3380: PPE not provided.(not allowed to wear hats in summer to shade face, ears, etc.). 5166: Chemicals/Automotive cans not disposed of properly.Alleged Hazards: 16, Employees Exposed: 1
