1. Plastic shower curtains are not in place. 2. Employees on first shift are not wearing masks. 3. There is mold on the wall in the women's restroom. 4. There are only two toilets for 150 employees. 5. One of the toilets in the women's restroom is hard to flush; valve problem. 6. The back room is opening for more production, however, there is mold on the back wall. 7. Also, in the back room, the ventilation is bad. In the past, employees in this area have developed pneumonia, bronchitis, and influenza. 8.
