1. In the Broadway Warehouse, Room 6, multiple storage racks are in disrepair with potential for falling over or collapsing under the weight of materials stored on them, exposing employees to struck by hazards. The items in disrepair on the racking may be but not limited to: 1. Punctured racks from forklift forks; 2. Racks not bolted to the ground; 3. Twisted racks. 2. Powered industrial truck operators are not provided training or certifications by the employer prior to operating powered industrial trucks. 3. The employer is not implementing adequate safety practices to protect its employees from the spread of SARS-CoV-2 such as, but not limited to providing face masks to all employees and visitors for use while at work.Source: Osha.gov | Receipt Date: 2020-12-10
