1. An effective Infectious Disease Preparedness and Response Plan has not been developed or implemented after four COVID-19 cases were confirmed in the last week, including but not limited to, temperature check and wellness screening. Masks are not worn systematically. 2. Employees exhibiting symptoms of COVID-19 are going to work exposing other employees to the virus. 3. Contact tracing has not been conducted with employees that has been in close contact with positive COVID-19 cases so they either could quarantine or get tested. 4. The employer has not provided adequate sanitization at the facility, including common areas and work areas, after confirmed COVID-19 positive cases.Alleged Hazards: 4, Employees Exposed: 35
