1. On the 4th Floor, Nurses are not consistently wearing masks for the duration of their shifts to protect them from contracting covid-19. No masks are worn at charting stations. 2. In the breakrooms, lunchrooms, and locker rooms, there is no social distancing being practiced. Nurses' stations also are not practicing social distancing. 3.
