1. Social distancing guidelines are not being followed or enforced in the facility to prevent the spread of COVID-19. Employees are working within six feet of each other and patients. 2. Cleaning and sanitation supplies are not being provided to the staff. 3. Employer is allowing the employees to go in work in other facilities who have COVID-19 positive patients and then return to their primary worksite without requiring those employees to wear masks.
