ITEM 1: The air conditioners in every patient room on the medical side leak water creating a fall hazard. ITEM 2: Employees do not have proper cleaning supplies to clean the facility effectively of prevent virus/infections. ITEM 3: Employees are not supplied with personal protection equipment. ITEM 4: The patient's beds do not lock in place creating a safety hazard for the patient and the worker whey are in need of changing diapers and linen. ITEM 5: The wheel chairs are in poor/barely workable condition and are unsafe to transport patients. Location: Throughout the facility.Alleged Hazards: 5, Employees Exposed: 20
